Appointments are required – to request space for your pet(s) please complete the form below. You must submit one form for each pet individually.

Clinics will be held at the Salt River Ballfields at 1839 N Longmore Rd OR at a location within 30 minutes of the Salt River Pima-Maricopa Indian Community. Your clinic location will be confirmed with you when our team reaches out to schedule. Pets are typically dropped off early in the morning, between 7:00 – 8:00 AM and picked up later in the day.

• All registered Tribal Members in Arizona are eligible. Tribal ID required at check in.
• Appointments include wellness exam, spay or neuter surgery, vaccines, microchip, pain medication to take home, and a surgical cone.
• Services are FREE, with a non-refundable booking fee due at scheduling to confirm your appointment. Fees are:$25 per pet OR $25 for a litter of puppies or kittens under 1 year old (from the same litter)
• Dogs & Cats must be at least eight weeks old AND at least 3 lbs on the day of surgery to be eligible.

NOTE: This form has changed for November and December 2026.
